I have a question regarding a client I am screening at the moment whom I have hit a brick wall with!
He has no LLD, and no problems with Gillets/ Downings. However, 4 Sign is +ve on both sides (and has been since the beginning) and whilst shoulder screens have improved dramatically, the pelvis is not moving. The piriformis is clearly very tight, and whilst 4 sign does bring about a slight change in knee height it slips straight back. is it possible that he needs soft tissue work day in day out for a few weeks before we see this improve? We've tried everything (soft tissue/ trigger pointing the piriformis then restesting 4 sign doesnt bring about any change) but he is someone who has always done alot of sport and has a lot of muscle mass. Therefore, perhaps one session of soft tissue isnt enough, and we just aren't working on it frequently enough? Slight hinge in QL on both sides, but again the major issue seems to be 4 sign only. TT is -ve.
We've tried anti -spasm and triggering off the upper body to see if it has a knock on effect to the pelvis but no result. Sciatic nerve doesnt seem to be a problem (90 degrees range on both legs) Hence I am stuck!

Have you tried the hip flexor release yet?
