Saw a client at the weekend who was complaining of severe lower leg pain and foot pain which appears to have been brought on - initially - from trying Brooks trainers stopping over-pronation. I observed NO over pronation! Screen summary: - Significant Neuro Dynamic issues with both sciatic and median/ulna nerve sliding - Significant lack of mobility in rotator cuff - couldn't raise either arm to horizontal for tests - Marginal or no movement in the pelvis - Significant tightness in hip external rotators / piriformis - Apparent leg length discrepancy demonstrated (2-3mm longer on RHS today) - Very tight hallucis extension on both feet - Significant supination resistance on both feet - Quad dominance evidence in both legs although no vagus knee evident - Slight tightness in RHS Pec Minor - Sub Talar motion was fine on testing - Hip flexors fine on testing OK. Cutting to the chase. VERY acute sciatic tension. Very acute median and ulna nerve tension. But also VERY acute Piriformis tightness. 4-sign aggravates Sciatic reaction in foot and calf. Wondered what peoples views are on correction priorities here? Happy to discuss in more detail on email or phone. Interestingly this client is a paediatric neuro-rehab Physio!!! Thanks Simon

I should add that on manual investigation, the pain was most definitely coming from Flexor hallucis longus

Hi Simon, it might be an idea to mail Rachel to alert her to this question, I'm not sure how often they check the forum.

If it were me, I'd trigger point piriformis and/or anti-spasm in an alternate position (standing or lying) and then try the least harsh sciatic mobs to see what happened then...

Hi Alex - thanks for replying.  I did email Rach, Jo and Martin on this one. 

I concur with your suggested approach. I've never come across a situation where 4-Sign aggravated a sciatic problem so dramatically.
